Terminate or Archive Providers

To mark a provider terminated or archived in the system, follow the steps below:

 

A Leave of Absence or Termination can be tracked from the Provider Status icon or the "Terminate" link.  Once the provider's Leave of Absence or Termination has been entered or the provider is terminated, this will also result in the provider's record being archived.  The steps for performing terminations or leave of absence are performed in the same manner.

  1. Search for Provider's partial name.  Once the Provider Search Results appear, click the Terminate link.

 

 

 

  1. To Terminate the provider, click on the link to the right of the provider's name. The following page will display.

  1. Enter a Termination Date.

  2. Select a Termination Reason from the drop-down menu.  
     

    1. If the reason you need is not available, select "Add New".

  1. Type any comments in the "Notes" box.

  2. Select the button.

  3. The provider is now marked "Terminated" in the system and you will return to the “Provider Search Results” page.

 

Note:  The following is for multi facility providers.

 

 

 

10.    When a provider is shared across multiple facilities, and the provider happens to be archived in one of the facilities, the facility will turn red in the pick list.
