Adding the Provider's References

Based on the provider’s application, the provider will supply three or more references and reference letters with the application packet.  To add the provider’s reference information the specialist will perform the following steps.

  1. Select the   option from the main menu.

  2. From the drop-down menu select "Provider Data".


     

  3. From the "Provider Data" drop-down select "References" from the "Experience" option.

     

  1. The "References" list for the provider will display.

  1. Select the button to open the “References” page.

  1. Enter the full name of the reference or select the search icon to open the “Provider Search” window.

     

  2. Enter the search criteria and select  to view a list of providers meeting the search criteria.


  3. From the list of providers select the  radio button to the left of the name to choose the provider as a reference. The following selection screen displays.

  4. Select a license by clicking on it. One must be chosen even if there is only one to choose from.

  5. Select a board/specialty.

  6. Select the Enter button to auto-populate the "References" page.

     

  7. Make any necessary changes or updates then select the Add button to add the reference to the provider.

  8. From a blank "References" page enter the following information. The full name of the reference for the provider as supplied on the provider’s application.

  9. Enter the title of the reference (i.e., MD, Director).

  10. Enter the specialty of the reference, where applicable.

  11. Enter the relationship of the reference to the provider.

  12. Enter the date the relationship of the provider to the reference began.

  13. Enter the date the relationship of the provider to the reference ended.  (If the relationship to the reference is on-going then leave the date blank).

  14. Enter the reference facility (multi facility users only!).

  15. Enter the references institutional affiliation (institutional affiliations can be education or hospital affiliations).

  16. Enter the address information for the institutional affiliation.

  17. Select the Add button to add the reference to the provider.

  18. Repeat the steps above to add all of the provider's references as listed on the application.

Edit References

  1. To edit an existing reference select the radio button next to the reference's name to open the "References” page.

  2. After all edits have been completed select the  button to save the information and return to the main “References” page.
